Notable examples include the 2003 Big Thunder Mountain derailment resulting in fatalities, the 2009 monorail collision leading to a fatality, the 2000 Space Mountain brain injury incident, and the 2013 Radiator Springs Racers collision causing injuries.

On July 5, 2009, a serious incident occurred at Walt Disney World Resort in Orlando, Florida, involving a collision between two monorail trains on the resort's monorail system. The crash resulted in the death of one of the monorail operators.The collision occurred early in the morning after one monorail train backed into another train that was stationary at the Ticket and Transportation Center station.
